• Application Fee ($10.00 U.S. Citizen - $25.00 International)
• Background Policy Form
• Concordia Health Form
• Immunization Record
• Official High School Transcript
• Official Transcript(s) of all Previous College Work
• Official ACT or SAT Test Scores (if taken)
• Housing Request Form (dormitory residents only)

International student files should also include the following:

• Affidavit of Support
• Proof of Health Insurance
• Copy of Visa and I-94 (front and back)
• Copy of I-20 if Transfer (If student has attended another institution in the U.S.)Evidence of the ability to read, write and understand the English language.
• Test of English as a Foreign Language (TOEFL) score of 500 or 170 computer, if the language of instruction is not English
